Adicciones: “Es cada vez más frecuente el policonsumo de drogas entre los jóvenes”
Un flagelo social siempre presente. Advierten que la edad de inicio es un indicador de consumo problemático. La cocaína es la droga ilegal de mayor consumo entre los pacientes de la Comunidad Terapéutica del Hospital Iturraspe, aunque de un tiempo a esta parte notan que "la gente que acude ya lo hace consumiendo más de una sustancia".

Cada 23 de septiembre se conmemora en todo el país el Día de la Lucha contra las Adicciones" en razón de conmemorarse en esa fecha el fallecimiento del doctor Ricardo Gutiérrez, médico y poeta, autor de una vastísima y sentida obra literaria, quien fuera en vida principal impulsor del Hospital de Niños que en la actualidad lleva su nombre.
La fecha pone en foco un flagelo social siempre presente en Argentina y en el mundo: el consumo de drogas. San Francisco no es ajena a esta realidad, ya que estas sustancias -tanto legales como ilegales- pueden conseguirse "a la vuelta de la esquina" y van deteriorando la vida de cientos de jóvenes y adultos.
Desde la Secretaria de Prevención y Asistencia de las Adicciones de la provincia alertan que la edad de inicio es un indicador de consumo problemático. La cocaína es la droga ilegal de mayor consumo entre los pacientes de la Comunidad Terapéutica del Hospital Iturraspe, aunque de un tiempo a esta parte notan que "la gente que acude ya lo hace consumiendo más de una sustancia".
En la provincia de Córdoba existe una red de contención y abordaje de la problemática de las adicciones a través de la Red Asistencial de las Adicciones de Córdoba (Raac) enmarcada en el Plan Provincial de Prevención y Asistencia de las Adicciones, lanzado por el gobernador Juan Schiaretti en diciembre de 2016.
La Raac tiene cuatro niveles de tratamiento. El primero de ellos aborda la promoción de la salud, la prevención del consumo de sustancias y la atención primaria en los centros preventivos. El segundo nivel lo ofrecen los centros asistenciales, donde se brinda un tratamiento ambulatorio especializado, multicomponente, que cuenta con Terapia individual, Terapia familiar, Terapia grupal, Abordaje psicofarmacológico, Terapia Familiar y Cesación Tabáquica. El nivel tres de la RAAC se define como el nivel más intensivo de tratamiento, está representado por aquellos establecimientos que cuenten con todos los recursos de un hospital general de agudos o de un hospital especializado en psiquiatría, para realizar desintoxicaciones derivadas del consumo de sustancias. Por último, el cuarto nivel es el de máxima complejidad destinado a la rehabilitación y reinserción de las personas con problemáticas asociadas al consumo de drogas. La provincia cuenta con dos Comunidades Terapéuticas Públicas Profesionales ubicadas en Santa María de Punilla y en San Francisco.
La primera de estas Comunidades funciona desde fines de 2018, en Santa María de Punilla, mientras que en San Francisco se inauguró la segunda Comunidad Terapéutica Pública Profesional en marzo de 2019, en nuestra ciudad.
En este centro de San Francisco actualmente hay 21 personas recibiendo tratamiento en internación. Desde su inauguración han pasado por allí 406 personas siendo el tratamiento promedio por paciente de una duración de 63 días y la edad media es de 30 años. Cabe destacar que junto a la Comunidad Terapéutica, en nuestra ciudad funciona un dispositivo ambulatorio que, en lo que va del año, lleva atendido 281 pacientes entre los cuales se llevaron a cabo unas 4.500 prestaciones.
"Tras la pandemia, sabíamos que iba a crecer la cantidad de personas que vendrían a buscar ayuda", dijo Gigena Parker.
Actualmente en el departamento San Justo hay cerca de 12 centros terapéuticos pertenecientes a los dos primeros niveles de la Raac que son los que se encargan de hacer prevención y de ofrecer tratamientos ambulatorios, tanto para personas que presentan problemas vinculados al consumo de sustancias como para sus familiares o seres queridos.
Darío Gigena Parker, secretario de Prevención y Asistencia de las Adicciones de la provincia de Córdoba, aseguró a LA VOZ DE SAN JUSTO que tras la pandemia de Covid-19 desde advirtieron "un incremento en la demanda" del sistema de abordaje interdisciplinario para el tratamiento de las adicciones.
En ese marco, el médico psiquiatra especialista en adicciones condenó el acceso al tabaco y el alcohol a edad temprana como los elementos que luego terminan derivando en el consumo de sustancias más peligrosas como el caso de la marihuana y la cocaína.
-¿Qué lectura hace en relación al momento por el que se transita en torno a las adicciones?
Estamos en un momento que sabíamos que iba a traer un aumento en la demanda como en el resto de los servicios de salud. Luego de la etapa más cruda de la pandemia, sabíamos que iba a crecer la cantidad de personas que vendrían a buscar ayuda sumado al impacto emocional y social que se reflejó en un aumento en la demanda de nuestro servicio. Hoy la Red Asistencial de las Adicciones de Córdoba (RAAC) está cubriendo 110 centros en distintos puntos de la provincia con dispositivos de internación en centros de mayor complejidad del ámbito de la salud pública como el que existe en San Francisco.

- ¿Cuál es la sustancia que más se consume entre las personas que llegan a la Comunidad Terapéutica?
El Observatorio Argentino de Drogas junto con el Indec está llevado adelante la Encuesta Nacional de Drogas y los datos estarán disponibles en mayo de 2023. Los datos que tenemos hasta el momento nosotros, reflejan población clínica y en ese contexto es cada vez más frecuente el policonsumo de drogas entre los jóvenes. La gente que acude a nuestros servicios ya lo hace consumiendo más de una sustancia. El motivo más frecuente suele ser el consumo de cocaína y sus formas de consumo como la variante que se fuma, así como también el crack. La cocaína fumable es mucho más adictiva y provoca un deterioro muy rápido de la persona.
-Preocupa el consumo de cocaína. ¿Y el alcohol?
Sí. La cocaína genera más preocupación porque las personas que la consumen de inmediato empiezan a ver que no la manejan y tienen muchos problemas. Eso hace que tomen conciencia del daño mucho más rápido y los lleva a pedir ayuda. En segundo lugar el alcohol y la marihuana. En el caso del alcohol, usualmente viene añadido al consumo de tranquilizantes pero esas personas tienden a consultar a edad adulta.
-¿Cuál es la puerta de entrada más directa al consumo de otras drogas llamadas "duras"?
Sin dudas que el tabaquismo o el alcoholismo a edad temprana. Preocupa la edad de inicio tanto en el consumo de alcohol como de tabaco porque ese es el indicador de mayor riesgo, más que la droga en sí misma.
-¿Cuál es la edad promedio de los jóvenes que se inician en el consumo de alcohol o tabaco?
En el caso del alcohol los datos oficiales marcan un promedio de edad de 15,5 años. Nosotros creemos que es 13,5 y esa edad varía de acuerdo a dónde se encuentre. Este es un dato que preocupa porque a partir de allí comienzan a experimentar con otras sustancias.
-¿La mayoría de los tratamientos que se hacen en la Comunidad Terapéutica son ambulatorios o también hay personas internadas por distintos períodos de tiempo?
Por lejos, hay muchas más personas con tratamientos ambulatorios. De hecho, en este momento hay 21 personas internadas y 300 con tratamientos ambulatorios.
¿Una nueva Ley de Salud Mental? Los adictos "han sido muchas veces estigmatizados o rechazados por el sistema de salud. Es importante poder contar con la atención de los hospitales", manifestó Gigena Parker.
-¿Qué opina sobre el debate en torno a cambios en la Ley de Salud Mental votada en 2010, que dificulta las internaciones, supeditándolas a la voluntad del paciente?
En este momento hay muchos proyectos. Uno de ellos propone separar el capítulo de las adicciones del resto del contenido de la ley de Salud Mental. Otro pretende modificar algunos artículos. En términos generales, la problemática de las adicciones requiere una particularidad en su análisis. Por otro lado, es importante rescatar el espíritu de la decisión de que la atención se realice en hospitales polivalentes para que la persona que tiene un problema por consumo de cocaína no esté en un lugar fuera de la atención médica general que tiene que recibir cualquier sujeto porque han sido muchas veces estigmatizados o rechazados por el sistema de salud. Es importante poder contar con la atención de los hospitales y por eso la Comunidad Terapéutica pública de San Francisco está en el predio del Hospital Iturraspe.
Además, hay cosas que mejorar, sobre todo en lo que se refiere al presupuesto en materia de Salud. Córdoba ha dado un paso importante en ese sentido, porque no tenía esta red de posibilidades mientras que hoy tenemos dos comunidades terapéuticas públicas y pronto habrá una tercera en funcionamiento. El problema crece de manera lamentable, pero aunque esto no pasara, igualmente hay que generar espacios de atención porque hay un enorme porcentaje de personas con adicciones que no están en tratamiento, no consultaron. Nosotros queremos ir a buscarlos, ofrecerles un tratamiento eficaz y llegar a los familiares para que esa personas pidan ayuda.
